ACCEPTING NEW REFERRALS FOR CASE MANAGEMENT ONLY

NOT ACCEPTING Children's HCT service referrals at the moment.
We currently accept MaineCare.
Referral Process:
Upon receiving your referral form, we will contact you to complete a prescreening over the phone.
We will answer any questions you have and with your consent proceed to complete an intake with full assessment.
We will then connect you with a community care specialist/mental health professional who will work with you to explore your goals and create your individualized treatment/ service plan.
