Case management supports individuals in successfully living in the community, minimizes the need for hospitalization, supports transitions to independence, and enhances his or her quality of life. (Ex: finding or maintaining housing, identifying and assessing community resources, staying connected to providers/appointments)

Independent Living Skills (Skills Development Services):

offers in-home support for adults living with mental/behavioral health challenges to strengthen necessary skills needed to live independently in their community of choice (Ex: practicing coping skills, learning to navigate transportation system).

Outpatient Services (Counseling):

are delivered by licensed mental health professionals, who work with clients on developing healthy copings, healthy relationships and a healthier life for themselves.

Home and Community Benefits (HCB) for the Elderly and for Adults with Disabilities :

these are in-home personal care and other services, designed as a package, to assist clients to remain in their homes, or other residential community settings, and thereby avoid or delay institutional nursing facility care. (Ex: laundry,  bathing, dressing, shopping, medication reminder, etc.)

CHILDRENS' SERVICES:

Home and Community-Based Treatment (HCT):

HCT is an intensive family-based service which is available to children, 0-21 years of age with particularly challenging behaviors. An HCT clinician works with all members of the family to address problematic behaviors.

A BHP (Behavioral Health Professional) will also work with the identified child and the parents to practice the skills the family is learning from the clinician. Therapy is most effective when a partnership is made between children, parents/caregivers and providers, therefore parent participation with both the HCT clinician and BHP is required.

This service is offered several hours per week within your home; may also occur at the child’s school or within the community.

Children's Outpatient Therapy:

delivered by a licensed clinician provides a supportive environment in which children/youth can learn healthy coping skills and self-awareness.
